No 1 of 2006 - Coroners Rules 2006 - Forms

I, Arnold George Shott, Chief Magistrate of Tasmania, make and publish the following Practice Direction pursuant to s.15AA of the Magistrates Court Act 1987 with regard to the practice and procedure of the Magistrates Court of Tasmania in the exercise of its jurisdiction pursuant to the Coroners Act 1995 (“the Act”) and Coroners Rules 2006 (“the Rules”) :

The following Forms have been determined as appropriate for use in investigations and inquests conducted pursuant to the Act and Rules:

  • Report of Death for the Coroner
  • Direction for Autopsy
  • Certificate Authorising Disposal of Human Remains
  • Summons to Attend as Witness or Produce Documents
  • Warrant of Apprehension of Witness
  • Coroner’s Authority to Police Officer or Authorised Person
  • Record of Investigation into a Fire
  • Record of Investigation into an Explosion
  • Record of Investigation into a Death (without Inquest)
  • Record of Investigation into a Death (with Inquest)
  • Authority for Release of Property
  • Exhumation Order
  • Notice Restricting Access to Place where Death Occurred
  • Notice Restricting Access to Place where Fire or Explosion Occurred
